Bacteriostatic water is the unsung hero of peptide reconstitution. Bad BAC water can contaminate an otherwise perfect vial. Here's my comprehensive sourcing guide, updated for 2026. What is bacteriostatic water? Sterile water with 0.9% benzyl alcohol as a preservative. The benzyl alcohol inhibits bacterial growth, allowing multi-use over 28-30 days after first puncture. DO NOT confuse with: - Sterile water (no preservative — single use only) - Normal saline (0.9% NaCl — different product) - Distilled water (non-sterile — never inject this) Verified Sources (medical-grade, USP): 1. Hospira/Pfizer 30mL vials — The gold standard. Available on Amazon for $10-$14/vial. Make sure the seller is a verified pharmacy supplier. Look for the NDC number (0409-1583-01). Check the expiration date on arrival. 2. Bacteriostaticwater.com — Dedicated supplier. $9.99 for 30mL. They provide a COA for each batch. I've used them for 2 years without issue. Ships from Nevada, arrives in 2-4 days. 3. Medical Supply Depot — $7.99 for 30mL generic USP BAC water. No brand name but meets USP specifications. Budget option that I've had independently tested — benzyl alcohol concentration was 0.87% (within acceptable range of 0.9% ± 10%). 4. Your local pharmacy — Many pharmacies stock Hospira BAC water behind the counter. No prescription needed. Usually $12-$18 for 30mL. Advantage: you can verify the lot number and expiration in person. Thank you for this. Couple of follow-up questions: 1. I ordered BAC water from Amazon and it arrived in a padded envelope in July — is it still good? It wasn't cold-packed or anything. 2. How much BAC water do I actually need per 5mg semaglutide vial? I've seen recommendations ranging from 1mL to 2.5mL and I'm confused about what's correct. 3. Does the needle gauge matter when drawing BAC water? I have 30g x 1/2" insulin syringes for injection but those seem really thin for drawing from a BAC water vial.

Great questions: 1. Heat exposure: BAC water is fairly heat-stable because it's just water + benzyl alcohol. Brief exposure to summer temps during shipping is unlikely to cause issues. The benzyl alcohol doesn't degrade easily. However, if the vial was exposed to extreme heat (140°F+) for extended periods, or if the seal looks compromised, discard it. Generally, a padded envelope shipment in July is fine if it wasn't sitting in a mailbox for days in Arizona. 2. Reconstitution volume: This is about concentration, not chemistry. The semaglutide will dissolve fully in any reasonable volume. Common concentrations: - 1mL added → 5mg/mL (concentrated — small injection volumes, harder to dose precisely) - 2mL added → 2.5mg/mL (most popular — good balance of precision and volume) - 2.5mL added → 2mg/mL (more dilute — easier to measure small doses during titration) For a starting dose of 0.25mg at 2.5mg/mL, you'd draw 10 units (0.1mL). At 2mg/mL, you'd draw 12.5 units. Either works. I prefer 2mL for the math simplicity. 3. Needle gauge: You can draw BAC water with a 30g needle, but it's slow and frustrating. Better approach: use a separate 25g or 27g needle to draw the BAC water and inject it into the peptide vial, then use your 30g insulin syringe for the actual injection. Or buy some 1mL luer-lock syringes with 25g needles specifically for reconstitution. They're about $15 for a box of 100.

Adding a data point on the Medical Supply Depot BAC water. I sent a sample to a friend who works in a compounding pharmacy and has access to USP testing equipment. Results: - Benzyl alcohol concentration: 0.91% (spec: 0.9% ± 10%, so 0.81-0.99% acceptable) — PASS - pH: 5.4 (spec: 4.5-7.0) — PASS - Particulate matter: None visible under 10x magnification — PASS - Sterility: Not formally tested (requires 14-day incubation per USP <71>), but my friend did a quick agar plate inoculation — no growth at 48 hours For $7.99, this stuff is perfectly fine. The brand-name Hospira is the safe choice, but the generic USP products from reputable suppliers test well. One note: I've tested two different lots from Medical Supply Depot over the past year. Both were within spec. Consistency matters and they seem to deliver.

RN/NP weighing in. A few critical safety points on BAC water handling: Technique matters more than source. Even perfect BAC water becomes contaminated if you: - Touch the rubber stopper without swabbing it with alcohol first - Use a syringe that's been uncapped and sitting open - Draw from the vial in a dusty or humid environment - Reuse syringes (never, ever do this) Proper reconstitution procedure: 1. Wash hands thoroughly 2. Swab the tops of BOTH vials (BAC water and peptide) with 70% isopropyl alcohol pads 3. Let the alcohol dry (30 seconds — this is the part people skip) 4. Draw the desired volume of BAC water 5. Inject slowly into the peptide vial, aiming the stream at the SIDE of the vial, not directly onto the powder 6. Gently swirl — do NOT shake. Semaglutide can denature if agitated aggressively 7. Wait until fully dissolved (usually 1-3 minutes for semaglutide) 8. Store reconstituted vial in the refrigerator (36-46°F / 2-8°C) After reconstitution: Use within 28-30 days. Mark the date on the vial with a Sharpie. If the solution becomes cloudy or you see particulate matter, discard it. BAC water quality is important, but aseptic technique is what keeps you safe. I've seen people buy the most expensive BAC water and then reconstitute on their kitchen counter next to a simmering pot. Please use common sense.
