I know this thread is old but I found it through search and wanted to add my experience with FDA compounding since this was the most comprehensive discussion I could find.
7 months later, the information in this thread still holds up. My results have been even better than expected based on this discussion.
For anyone else finding this through search: the advice here is solid. Some of the vendor information may be outdated.

+1 to sarah.morrison. Especially the point about "The first 6 weeks are the hardest..." — I have seen the same in my own experience with FDA compounding.
