I've been tracking compounded semaglutide prices across multiple channels and the market has shifted significantly since the FDA's updated guidance in January. I'll start. I have a UnitedHealthcare PPO through my employer. They added Wegovy coverage in October 2025 with a $75 copay. So I'm paying $75/month for brand name. Before that, I was paying $189/month through Empower with a Push Health prescription. The catch? UHC requires a prior authorization, BMI of 30+, and documented "failure" of diet and exercise. My PCP had to submit a letter. Took about 3 weeks to get approved. Also requires quarterly check-ins. For people without insurance coverage, the compounding route is dramatically cheaper. At $189/month vs $1,349, you're saving over $13,000 annually. Even the telehealth bundles at $299/month save you $12,600/year.

Important update on Empower Pharmacy β€” they just raised prices effective March 1st. The 5mg/2mL vial is now $195, not $175. I confirmed this when I placed my refill order yesterday. They cited "increased API costs and regulatory compliance expenses." Hallandale is still at $155 as of last week. That makes them the cheapest 503B option I'm aware of right now. Also worth noting: if you're on a maintenance dose of 0.5mg/week, a single 5mg vial lasts you 10 weeks. At $155/vial from Hallandale, that's $15.50/week or about $67/month. Even at the higher doses (2.4mg/week), a 5mg vial lasts just over 2 weeks, so $310/month. Scale your cost expectations to your dose.

Good breakdown Dave. Let me add the per-mg math for people titrating up: At $155/5mg vial (Hallandale): - 0.25mg/week = 20 weeks per vial = $7.75/week ($33.50/mo) - 0.5mg/week = 10 weeks per vial = $15.50/week ($67/mo) - 1.0mg/week = 5 weeks per vial = $31/week ($134/mo) - 1.7mg/week = ~2.9 weeks per vial = $53.10/week ($230/mo) - 2.4mg/week = ~2.1 weeks per vial = $74.30/week ($322/mo) At $99/5mg lyophilized (domestic research): - Same dose schedule but roughly 36% cheaper across the board - 1.0mg/week works out to about $19.80/week ($86/mo) The tradeoff is clear: research-grade saves money but you're reconstituting yourself, there's less regulatory oversight, and COA quality varies. Pharmacy-grade costs more but you get sterility testing, proper BUD dating, and a licensed pharmacist compounding it. For reference, I was on brand Ozempic before my insurance dropped coverage. Was paying $25 copay, then suddenly got hit with $935/month. Switched to Hallandale compounded and haven't looked back.

This is incredibly helpful. I'm just starting out β€” my doctor prescribed 0.25mg/week for the first 4 weeks, then titrating up. A few questions from a newbie: 1. For the telehealth bundles (Ro, Henry, etc.) β€” do they adjust the dose as you titrate, or do they send the same vial concentration and you just draw more? 2. Is there a meaningful quality difference between $155 Hallandale and $99 research-grade? The COA threads here seem to suggest both test well. 3. How much does bacteriostatic water cost on top of the vial price for lyophilized? Nobody seems to include that. Trying to budget this out for 6 months while I figure out insurance.
