I cannot with these sulfur burps anymore y'all. I'm on tirz 7.5mg, week 3, and I am BELCHING rotten eggs every 30 minutes. My husband literally left the bedroom last night. My coworker asked me if something died in my office. I'm not even exaggerating.

I've tried:

  • Pepto Bismol β€” helps for like 20 minutes then they come back
  • Eating smaller meals β€” doesn't matter
  • Gas-X β€” lol no
  • Cutting out eggs and dairy β€” nope still happening

PLEASE someone tell me this goes away because I love what this drug is doing for my appetite and weight but I cannot live like this 😭

Oh god I feel this in my SOUL. I had the exact same thing at 7.5mg. It was genuinely affecting my social life. Like I would avoid going to restaurants.

What finally worked for me (combination approach):

  1. Align probiotic β€” took about 5 days to kick in but made a massive difference
  2. Digestive enzymes with meals β€” specifically ones containing alpha-galactosidase
  3. STOP eating within 3 hours of injection β€” this was the biggest one. I inject at night now and don't eat after 6pm on injection day
  4. Peppermint tea β€” sounds like BS but genuinely helped

The burps mostly resolved by week 6-7 for me. Your gut microbiome adjusts. Hang in there πŸ€’β†’πŸ˜Š

Gastroenterologist here. The sulfur (hydrogen sulfide) eructation is one of the most common complaints I hear from my tirzepatide patients, probably affecting 15-20% to some clinically meaningful degree.

The mechanism is likely related to delayed gastric emptying β€” food sits in the stomach longer, sulfur-reducing bacteria (particularly Desulfovibrio species) produce Hβ‚‚S gas from dietary sulfur-containing amino acids (methionine, cysteine). Tirzepatide slows gastric motility significantly, especially during dose escalation.

Evidence-based interventions that actually have some data behind them:

  • Bismuth subsalicylate (Pepto) β€” binds Hβ‚‚S directly but short-acting, as you've discovered
  • Probiotics β€” can shift gut flora away from sulfur-reducing bacteria. Lactobacillus and Bifidobacterium strains have the most support
  • Reducing high-sulfur foods β€” cruciferous vegetables (broccoli, cauliflower, cabbage), alliums (garlic, onions), and high-protein meals are the main dietary sources. Dairy and eggs are less impactful than people assume
  • Simethicone (Gas-X) β€” not effective for sulfur burps specifically. It breaks up gas bubbles but doesn't address Hβ‚‚S production

It does typically improve within 4-8 weeks as the gut adapts. If it persists beyond dose stabilization, consider a short course of rifaximin β€” talk to your GI or PCP.
