+1 to hyun_seoul. Especially the point about "This is incredibly helpful..." — I have seen the same in my own experience with Vitamin B12 monitoring .

I have managed ~150 patients on GLP-1 therapy and this topic comes up frequently. What the data shows — and what I see in practice — is that proper titration prevents most adverse events.

For this specific question, I would recommend: reviewing the relevant clinical guidelines.

Want to share my personal experience related to Vitamin B12 monitoring anyone since I think it might help others in a similar situation.

I started my GLP-1 journey in March 2025 at 252 lbs with metabolic syndrome. My telehealth provider prescribed semaglutide 0.25mg to start.

Now, 14 months later: I am down 62 lbs, my labs have normalized, and my sleep apnea has improved dramatically. The Vitamin B12 aspect specifically was something that made a big difference in my journey.

This forum was my go-to resource throughout.

Just wanted to say — for anyone reading this Vitamin B12 thread who is feeling discouraged: it gets better. The first 6 weeks are the hardest. Hang in there. 💪
