I'm 5 months into semaglutide (currently 2.4mg), down 52lbs. Results have been amazing BUT... I'm losing hair. Like, a lot of hair. My shower drain is a horror show every morning. My ponytail is noticeably thinner and I can see my scalp in places I couldn't before.

I know people say "it's telogen effluvium from rapid weight loss, not from the drug" but does the distinction even matter when the result is the same? I'm a 35F and this is honestly making me consider stopping sema even though it's otherwise changed my life.

Has anyone dealt with this? Did it stop? Did your hair grow back? What helped? I'm borderline panicking tbh.

Please don't panic. I went through this exact thing and I know how distressing it is. Here's my experience:

I lost a LOT of hair starting around month 4, peaking around month 6-7. It was genuinely scary. But it did stop, and my hair has been growing back. I'm now at month 14 and my hair is about 80% back to normal — maybe even better because I'm healthier overall.

What my dermatologist told me: this is almost certainly telogen effluvium (TE), which is triggered by rapid weight loss, nutritional changes, and metabolic stress. It happens after bariatric surgery too — in fact, the rates are similar or even higher post-surgery. It's not unique to semaglutide.

The hair loss typically starts 2-4 months after a "trigger" event (starting rapid weight loss) and resolves 6-12 months after the trigger stabilizes (when weight loss slows or stops).

As a dermatologist who treats this regularly, a few points:

Telogen effluvium vs other causes: TE is by far the most likely explanation, but it's worth ruling out other causes, especially in a patient on semaglutide who may be eating significantly less:

  • Iron deficiency: Very common in women with reduced food intake. Check ferritin — you want it above 40 ng/mL for optimal hair growth, even though the "normal" range starts at 12.
  • Zinc deficiency: Also common with reduced intake. Easy to supplement.
  • Protein deficiency: If you're not eating enough protein (which is common when appetite is suppressed), your body will divert amino acids away from hair growth.
  • Biotin: Deficiency is less common but worth checking. 2500-5000mcg daily supplementation is safe.
  • Thyroid: TSH should be checked. Rapid weight loss can unmask or worsen hypothyroidism.

I'd recommend getting a full panel including ferritin, zinc, vitamin D, B12, and TSH before assuming it's "just TE."

In STEP 1, hair loss was reported in about 3% of semaglutide patients vs 1% of placebo. So while it is more common on sema, the absolute risk is still relatively low.[1]

I'm going through this right now too 😞 Month 6, down 44lbs. The hair loss started about 6 weeks ago. I went to my derm and she confirmed TE. Her recommendations:

  • Minimum 100g protein daily (I was only getting about 60g — oops)
  • Iron + vitamin C supplement
  • Biotin 5000mcg
  • Viviscal or Nutrafol supplements
  • Be patient

I also started using rosemary oil on my scalp and idk if it's doing anything but it makes me feel proactive lol

I lost about 30% of my hair volume between months 4-8. It was devastating honestly. But I'm now at month 18 and it's grown back fully. Like 100%. The new growth came in slightly different texture initially (curlier) but now it's normalizing.

Hang in there. It DOES come back. I know that's easy to say when you're not the one watching clumps fall out but it truly does resolve.
