Following this thread closely. I'm about to start sema and honestly the GI side effect stories have me nervous. Would it make sense to start BPC-157 before beginning semaglutide as a prophylactic measure? Or is that overkill?

Probably overkill. Most people tolerate the GLP-1 titration fine, especially if you follow the slow titration schedule (0.25 mg x4 weeks → 0.5 mg x4 weeks → etc). The GI effects are most common in the first few weeks at each dose level and often resolve with adaptation.

I'd say start the semaglutide, follow the standard titration, and keep BPC-157 in your back pocket if GI issues become persistent beyond the expected adaptation window. No point adding complexity and cost if you might not need it.

Also worth noting: the basics matter more than peptide stacks. Stay hydrated, eat smaller meals, avoid high-fat foods during titration, and don't eat to the point of fullness. These simple measures resolve GI issues for the majority of patients.

Adding some sourcing context since several people have DM'd me about this. BPC-157 is one of the more widely available research peptides. Most US-based peptide suppliers carry it in lyophilized form (typically 5 mg vials).

Key quality markers to look for:

  • Third-party CoA (certificate of analysis) with HPLC purity ≥ 98%
  • Mass spec confirmation of the correct molecular weight (1419.53 Da for the free acid form)
  • Endotoxin testing (LAL test) — especially important for injectables
  • Lyophilized white powder, not yellowish or clumpy

Reconstitute with bacteriostatic water (0.9% benzyl alcohol). A 5 mg vial reconstituted with 2 mL BAC water gives you 2500 mcg/mL — so 0.1 mL = 250 mcg. Store reconstituted vial refrigerated, use within 3-4 weeks.

Cost runs about $25-40 per 5 mg vial from reputable sources, making a typical 8-week cycle (250 mcg BID) roughly $80-120 total.

This is exactly what I needed — thank you all. Going to order a couple of vials, start with 250 mcg subQ BID, and run it for 6-8 weeks alongside my sema. Will report back with an update. Already feel better just having a plan. 👍

Good discussion. Pinning the reminder that BPC-157 is sold for research purposes only and is not FDA-approved for human use. Nothing in this thread constitutes medical advice. If you're experiencing significant GI symptoms on a GLP-1 RA, please consult your prescribing physician first.

That said, this is one of the more evidence-informed peptide discussions we've had. Carry on. 🧪
