I follow an almost identical protocol and can confirm it works. The two biggest game changers for me were (1) switching to bedtime injection and (2) the strict no-fat rule on days 1-3. The first time I ate a burger on day 2 post-injection I thought I was going to die. Now I plan my meals around the injection cycle.

One thing I'd add: avoid sugar alcohols (found in sugar-free candy, protein bars, etc.) during the high-nausea window. Erythritol, maltitol, xylitol — they're already hard to digest normally, and with delayed gastric emptying they ferment and cause bloating, gas, and worsen nausea dramatically.

The mental component is also important. During my worst nausea phases, I was catastrophizing — "this will never get better, I'll have to quit, the nausea will be this bad forever."

Reminder to everyone: in the STEP 1 trial, nausea peaked during dose escalation and decreased significantly after week 20, even at the full 2.4 mg dose. Your body adapts. The GLP-1 receptors in the area postrema desensitize over time. The nausea you feel at week 4 is NOT what you'll feel at week 20.

Give yourself permission to struggle while it's hard, but know it gets better for the vast majority of people.

Random but relevant — if you're taking metformin alongside a GLP-1 RA, the nausea can be compounded significantly. I was on metformin 1000 mg BID + semaglutide and the nausea was unbearable. My endo switched me to extended-release metformin and eventually dropped it entirely since sema was managing my glucose. Nausea improved about 60% just from that change.

Worth discussing with your prescriber if you're on both.

Great additions everyone. And yes, please share with your patients, doctor! The more people who have a concrete plan going in, the fewer will suffer unnecessarily or give up on a medication that could change their health trajectory.

The sugar alcohol point is excellent — I should have included that. Protein bars are such a go-to for people trying to hit protein goals on low appetite, but so many of them are loaded with sugar alcohols. Stick to real food during the nausea window.
