Look, I can handle the nausea. I can handle the constipation. But the SULFUR BURPS on tirzepatide are making me want to crawl into a hole and never interact with another human being.

For those who don't know β€” imagine burping rotten eggs. Now imagine that happening 30-40 times a day. In meetings. On dates. In elevators. My coworkers have noticed. I want to die of embarrassment.

I'm on 7.5 mg tirz, been having this since I moved up from 5 mg three weeks ago. PLEASE tell me there's a fix for this. 🀒

Oh friend, I have BEEN THERE. The sulfur burps are tirzepatide's special gift to humanity. They don't get talked about enough because they're so embarrassing but they are SO common.

What's happening: delayed gastric emptying means food sits in your stomach longer β†’ bacteria ferment it β†’ hydrogen sulfide gas β†’ rotten egg burps from the depths of hell.

What actually helped me:

  • Simethicone (Gas-X) after meals β€” won't prevent the gas but helps you pass it more quietly πŸ’€
  • Pepto-Bismol β€” the bismuth actually binds hydrogen sulfide. This was the single most effective thing for me. One dose before my biggest meal.
  • Avoiding trigger foods: eggs, broccoli, cauliflower, red meat, dairy, garlic, onions β€” basically anything high in sulfur compounds
  • Eating smaller meals so there's less sitting in your stomach fermenting
  • Probiotics β€” specifically ones with L. rhamnosus and B. lactis

Mine mostly resolved about 6 weeks after moving to 7.5 mg. Your gut adapts. But those 6 weeks were ROUGH.

Pepto-Bismol is legit the answer. I take it prophylactically before meals now and the sulfur burps went from constant to rare. Just be aware it turns your stool black (which is harmless but alarming if you're not expecting it) and don't take it long-term without talking to your doc because of the bismuth.

The sulfur burps are indeed related to bacterial fermentation in the setting of delayed gastric emptying. A few clinical pearls:

  1. Bismuth subsalicylate (Pepto-Bismol) is evidence-based for this β€” it directly precipitates hydrogen sulfide. Safe for short-to-medium term use but avoid if you're on blood thinners (it contains salicylate).
  2. If sulfur burps are accompanied by significant upper abdominal pain, bloating, and early satiety, consider getting evaluated for H. pylori β€” the bacterium itself produces hydrogen sulfide and GLP-1-induced slow emptying can unmask a previously subclinical infection.
  3. Some patients benefit from a short course of rifaximin (Xifaxan), a non-absorbed antibiotic that reduces gut bacterial overgrowth. This requires a prescription.

The good news: this side effect has one of the highest rates of spontaneous resolution. Most patients who stick with it find it subsides within 4-8 weeks at each dose level.

i literally thought I was the only one. I've been too embarrassed to even google this. I burped in my boss's face during a one-on-one and wanted the floor to swallow me whole. thank you for making me feel less alone in my sulfur burp journey πŸ˜‚πŸ˜­
